(LN) HIV isolates.

AB OBJECTIVE: Previous results obtained by PCR or coculture techniques have
shown higher viral burden in lymph node mononuclear cells (LNMCs) than
in peripheral blood mononuclear cells(PBMCs). The aim of this study was
to comparate the phenotype of HIV simultaneously isolated from PB and LN
in HIV infected patients (pts). METHODS: Patients :11 HIV-1 and 1 HIV-2
infected adult pts(CDC I: 1, CDC III: 8, CDCIVC2: 3). 9 untreated pts, 3
on Zidovudine (ZDV) or ddI therapy. Cells:PBMCs and LNMCs from LN
surgically excised for diagnostic purposes were obtained on the same
day. Virological studies: PB and LNMC load:ANRS consensus coculture
protocols performed on fresh samples. In vitro sensitivity to ZDV:
cell-free isolate sensitivity assay (ANRS consensus technique). SI
phenotype of isolates assayed on PBMCs and MT-2 cells. RESULTS:
Frequency of infected cells was one log greater in LNMCs compared to
PBMCs in the 12 pts investigated. PBMC and LN isolates recovered from
the 9 ZDV naive pts were sensitive to ZDV with similar IC50 and IC90
values. PBMC and LN isolates from 2 pts were ZDV sensitive after at
least 12 months of ZDV therapy. LN isolate from one IVC2 pt who had
switched one year before to ddI after 12 months of ZDV therapy, showed a
five-fold increase in IC90 values contrasting with an IC50 of ZDV
sensitive isolate. SI phenotype was found in LN isolates from 2 CDCIVC2
pts and 1 CDC III pt, while 9 LN isolates and all the 12 PBMC isolates
were NSI. CONCLUSION: The presence in the only lymph nodes of isolates
showing either reduced sensitivity to ZDV or SI phenotype suggests a
greater heterogeneity of LN isolates compared to PBMC isolates and these
data could contribute to our understanding of pathogenesis of HIV
infection.
